Femoroacetabular impingement (FAI) and labral tears are a common cause of groin pain in young active patients. These injuries can often go misdiagnosed for years as "hip flexor tendonitis." Appropriate Xrays and an MRI can identify the problem and if patients fail nonsurgical management hip arthroscopy is a great option for addressing their pathology. Picture 1 shows the pincer (acetabular sided spur) behind the labrum. After resection of the spur (pic 2) an anchor is placed... in the bone (pic 3) and the labrum is repaired (pic 4-5). We then identify the bump of bone on the femur (CAM lesion pic 6) and shave it away under Xray guidance (pic 7). It is important to hold on to the general fracture management training we receive even as subspecialists. This case is a reverse obliquity IT/subtrochanteric hip fracture in an older gentleman who fell in his home. The fracture is anatomically reduced under traction and the use of a percutaneous clamp. Through three small incisions a titanium nail and screw construct is placed under Xray guidance to lock the fracture in place and hold it stable while the bone heals! Last case of the week was a high school martial artist who over the last year tore BOTH ACLs! We performed staged ACL reconstruction and on the most recent there was a peripheral tear in the medial meniscus (picture 1). In picture 2 you can see the blood visible in the capsular side of the tear indicating this is a repairable tear in the "red-red zone." Picture 3 shows the final repair with 4 vertical mattress sutures. A combination of all-inside and outside-in techniques wer...e used to perfectly reduce this tear. A large portion of time spent with patients is educating them on the causes of their injuries. In my overhead throwing ahtletes there are many factors that lead to shoulder and elbow injuries but one cause I always hone in on is single sport specialization at a young age! Baseball pitchers especially will have much higher rates of SLAP tears and UCL tears playing a single sport year-round! Femoroacetabular Impingement (FAI) is a common cause of hip pain leading to labral tears. For patients that fail conservative treatment with NSAIDS, PT, and injections hip arthroscopy can be utilized to resect the acetabular (pincer) and femoral (CAM) impingement sources. Picture 1 shows mixed type FAI with both a pincer and CAM outlined in yellow. Picture 2 is a lateral view of the hip that shows the anterior extent of the bony bump on the femur. After hip arthroscopy pict...ures 3 and 4 demonstrate correction of both sources of impingement. This patient also had extensive labral tearing requiring a labral repair. In video 2 I demonstrate the Pivot-Shift examination which evaluates rotational stability of the knee. ACL tears create translational (anterior to posterior) as well as rotational instability of the knee. Without an intact ACL the knee will continue to buckle with running, cutting, and sporting activities. In video 1 I perform a Lachman exam where you can can see the tibia sublux anterior without a defined end-point. Very rarely is arthroscopic surgery indicated for degenerative arthritis of the knee. However, this 42 year old male has significant arthritis of the knee on Xray and reports no minute-to-minute pain. In fact, his biggest complaint was catching and locking sensations of the knee while working out at the gym!! Pic 1 and 2 show his arthritis and a large loose body in the lateral gutter of the knee. We found it and removed it arthroscopically (pic 3 and 4). In total we removed... four loose bodies from his knee the largest of which was about the size of a golf ball!
