EMS Documentation - EMS Charts Users. Common Errors that have been identified in PCRs #1 Who is the patient - Spelling is IMPORTANT! Name spelling is CRITICAL! Get in the habit of asking your patient or their family member, "How do you spell your name?" This is especially critical with reference to the last name. ("Smith" when your patient's name is "Smyth.") This is a case where "hooked on phonics" just won't do. In this day of electronic health records and computerized cl...aims for billing, the exact match of the name in a hospital database or in the insurance records will make all the difference in pulling up and matching key data connected to the patient you just transported. You don't get a pass on first names either. For example, computer databases will fail to match written name "Christa" if your patient's name is spelled "Krysta." #2 - Additionally, it is important to be careful you don't transpose (Reversing) first & last names #3 - Transposing month and day in birth date #4 - Finally, receiving destination; Albany Medical Center - Emergency Department (Do NOT use the Emergency Dept then Peds ED for Unit Received) v. Albany Med Massry Children's Center (Peds ED) We are utilizing Health Information Exchange) technology, which allows for easy, real-time bi-directional data sharing between the EMS staff and the hospital’s patient care team and electronic medical record (EMR) system. This seamless communication allows the hospital to access the PCR directly from their electronic health record, and your agency can access the hospital’s treatment details, outcomes, and billing information. We need your attention/assistance to improve the patient demographic information in your PCRs for many reasons. CONSTRUCTION UPDATE - On Saturday, Nov. 7, beginning at 4 a.m., Engineering and Construction will be installing equipment on the roof of C Building. This work will require a crane to be set up on the northside of New Scotland Ave. During this time, New Scotland Ave. will be closed between the intersections of Holland Ave. and Myrtle Ave. This closure applies to both vehicles and pedestrians. Flaggers will be on site to direct traffic. Pedestrians will have access to the h...ospital via the 40 New Scotland Ave. pedestrian bridge and the West Physician’s Entry. The Pillars and B Building Entries will remain closed. Pedestrians will be allowed on the south sidewalk of New Scotland Ave. to the entrance of 50 New Scotland Ave. to access the buildings and businesses within. Vehicular traffic wanting to park in the 60 New Scotland Ave. garages will be able to enter via Holland Ave. There will be access to the 40 New Scotland Garage via Veterans Way by showing your Albany Med ID to the flagger stationed at the Myrtle Ave. and New Scotland Ave. intersection.
