Oral Antal Fistula: always lurking around the corner, especially during molar extractions, is a perforation of the Schneiderian (sinus) membrane. Most small ones heal spontaneously. Larger ones need surgical closure/repair ... and that can fail too! ... leading to a communication between the mouth and the Maxillary Sinus. A methodical approach, meticulous planning and execution maximally controlled setting leads to excellent outcomes!
