VETERANS!!! On Saturday, February 13, 2021, the Northport VA Medical Center will be operating a Walk-In COVID-19 Vaccine Clinic for Veterans who are over 65 years old from 10 am to 1pm. No appointments required for this day only. Requirements:... Veteran must be over 65 years old Veterans must have been seen at the VA or through one of the VA operating clinics within the past 2 years. Veterans must be registered with the VA and have an active VA account to be eligible. Supplies are limited, and the vaccine will be distributed on a first come, first serve basis. Once the stock has run out, the walk-in clinic will close down. Appointments will be given for the 2nd dose at the clinic when you receive the vaccine. You will know immediately when to return after receiving your first vaccine The clinic is located at the Northport VA Medical Center Campus, located at 79 Middleville Road, Northport New York 11768. Please follow the signs and police vehicles to the vaccination clinic area.

People 75 and older Prior to receiving the vaccination, you must complete the New York State COVID-19 Vaccine Form. This form can be completed online and you will receive a submission ID, or you can fill out the form at your vaccination site. Individuals being vaccinated must bring proof of eligibility to the vaccination site.
