Listen Up, @invisalign patients. This is my amazing niece Bailey who just graduated from her PA program @southu in #savannah. She started with aligners not even 2 weeks ago, and as I’m watching her Class President address, I’m thinking, I BET she didn’t even take the aligners out for her brief address. And of course, she hadn’t! Now, I don’t expect everyone to have her superhuman abilities or confidence level but, c’mon, don’t tell me you can’t speak with your aligners in! When teeth are misaligned, they often absorb the light unevenly, which can exacerbate minor/moderate misalignment. Most #Invisalign cases have teeth looking amazing in 6 months or less (though more time might be required for final detailing). Have you heard the expression that kids grow into their teeth? This is my friend Jeff, who sent me this childhood photo. He did receive orthodontic treatment to correct his bucky overbite but even with a change in tooth position, can you see how his tooth size seemed so much larger as a child? That’s because tooth-size doesn’t grow, but head size does. For this reason, if I correct crowding in a child with braces or #Invisalign and the result leaves them with a slightly big or protrusive smile, future head growth will nicely balanced things. And if you extract teeth as part of the Orthodontics and leave the smile on the smaller size, future growth will shrink that smile further. #orthodontics #orthodontist #NYC #Newyork

Patient was missing both upper lateral incisors. With this type of stronger jaw profile, using #orthodontics to shift the teeth to close the space of the missing teeth would not work. Both braces and #Invisalign were used in treating this case. Most orthodontists, including the Dr Adam Goodman of 2015, would have this patient in braces for at least 15 months and probably longer. It would take a few months to even have clearance to comfortably place the lower brackets. Braces are not as linear as people thinks- a bracket breaks, a wire shifts, a bracket is imperfectly positioned, the wire distorts- it’s a puzzle with some ups and downs and unexpected changes. With #Invisalign, if you know how to set up an efficient plan, it goes in auto-pilot. This case is completed in 8 months! I am often asked if I can orthodontically close the space of the missing tooth the save patient an implant and at first glance, this case, with an anterior crossbite, might appear to set up well for that. My vision focused on the upper teeth, and you can’t broaden the upper arch and constrict the lower arch (with extraction) or you will end up with a poor occlusion. The green dots show how this patient’s lower right canine was impacted beneath the gum without available space, and the same tooth’s final position. But you can’t expand the lower arch to make room unless you do the same to the upper arch. I am careful to avoid overstating the effects of Orthodontics on more than just the smile. This patient had previous removal of 2 upper premolars and you can see how the lower front teeth are all pushed back to accommodate the fact that the upper teeth were moved too far back. So the treatment involved moving the upper teeth forward with Invisalign and opening up about 2.5 mm in each of the two (closed) extraction sites. This photo doesn’t show that, but what it does sho...w, to my pleasant surprise, is that the lips and surrounding muscle tone look enhanced. Most orthodontists agree that removing premolars can make the profile concave IF the space created from the extractions was excessive and fully closed. This post will resonate more with dentists and pedodontists. The second photo, without the colored dots, not the split screen, is a 13 year old who came in having been told for years it was too soon to see an #orthodontist and probably wouldn’t need anything. The dentist checking him every 6 months missed that there were still two primary canines in place way too long. The split screen colored dot photo shows a different patient seen at age 9. I saw the high canines (yellow) and we proceeded to remove teeth b c h i. It important to not only remove the primary canines but also the primary first molars because 5 and 12 are often wedged up against 6 and 11 respectively. Look how the teeth improved. These were taken 2 years apart.

We have been amazed by the result of Invisalign on kids age 7-9. SO much easier than a palatal expander (which still has a place when true bony expansion of the maxilla is needed). And young kids are starting to think Aligners are cool because they see more older kids with them. But some still want to express themselves with color. If I over-promised and under-delivered, Goodman Orthodontics would probably start more patients; but then we couldn’t run precisely on time like we always do, and we would have unhappy patients who are disappointed with their results. At the initial consultation, rather than keep it simple, and try to sell, i explain the detail: is the treatment functional or esthetic, what happens if a bad bite is left as it is, what aspects of the final result should you be prepared for, will you need to see a cosmetic or restorative dentist to change your tooth shape before or after your treatment.
