Officers Rescue Occupant Trapped in House Fire This morning at approximately 4:26 AM, Sheriff’s Deputies along with State Troopers New York State Police were di...spatched to a reported residential fire at 127 Cedarwood Boulevard in the Town of Van Buren. Sheriff’s Deputy Laura Collins arrived on scene within minutes and discovered heavy smoke and flames coming from the rear of the home. Not knowing if the residence was occupied, she began pounding on the doors and windows while yelling in an attempt to alert anyone inside. A woman eventually came to the front door and was escorted safely out of the burning home. The woman informed Deputy Collins that her 23-year-old son was still inside the home. Deputy Collins entered the home in an attempt to search for the male but was forced to evacuate due to the heavy smoke. After yelling multiple times, the male came to a second story bedroom window and informed Deputy Collins that he was trapped in the room. With the assistance from arriving Troopers, John Moretti and David Henrie, along with Deputy William Adamo, they worked together to stack a neighbors garbage bins directly underneath the males window where they were able to escort the male out the window and safely away from the home. Firefighters from the Lakeside Fire District and Baldwinsville Vol. Fire Company arrived on scene and were able to get the fire under control within 20 minutes, which caused significant damage to the residence. Deputy Collins, along with the two occupants of the home, sustained minor smoke inhalation and were medically treated at the scene. Onondaga County Fire Investigators were called to the scene and determined that the fire started by the careless disposal of a cigarette on the rear porch. Investigators also found that the home had no working smoke detectors. The American Red Cross is currently working with the homeowner on relocating them. Great job by all involved! Update from Dr. Cushman, Regional EMS Medical Director for Monroe County, City of Rochester and Monroe-Livingston Regional EMS Medical Director: "Let me first p...rovide some clarification as I have been corrected by a few officials and I am all about making sure facts are accurate. I was incorrect to say that EMS is off of Tier 1A. The governor’s press conference yesterday (12/9) and subsequent release drove home the fact that EMS is not considered in the same category as high risk health care providers specifically our colleagues in the ED and ICU. To me, this is unacceptable. However, as I said, words matter: Following the vaccination of 'high risk' health care workers, the priority will shift to all long-term and congregate care residents and staff, EMS and other health care workers. EMS practitioners, as skilled healthcare personnel working in the streets, are just as susceptible to illness if not moreso than our hospital colleagues working in the ICU and ED settings. Being in an ambulance that is about 80 cubic feet, for 30-90 minutes, well less than 6 feet away, and in a box with minimal ventilation is certainly high risk and has no equivalent in any other healthcare setting with all respect to my hospital-based colleagues. The message is simple. EMS should be considered as equals in this regard so that our 911 safety net is preserved. EMS is in Phase 1, but Priority 2 within that phase. Critical is that EMS are high risk HCP’s and should be in the group within the phase 1, priority 1 tier. Not in the next tier which will likely be weeks. Shame on us for not taking action sooner and educating the panel on the risks our personnel take every day with undifferentiated patients with respiratory conditions, intubating them in the field, and performing interfacility transports with critically ill COVID + patients from ED’s to ICUs. It will take many days, if not weeks to vaccinate even just EMS personnel as I am planning on 2 days after each cadre to make sure there are no side effects that pull people out we can’t afford a 10-15% sick out rate after each vaccination round else we will be without staff. We know that initial doses will require allocation (read rationing) however it is not clear who is advocating for EMS and how EMS even accesses vaccine as the last few hours have indicated from colleagues like you that vaccine availability and access for EMS does not appear to be consistent across the state. We recognize that we will simply not have enough vaccine for a number of months to get to everyone that wants it, and so having the same ability to ration doses as other high risk health care providers is critical for maintaining our workforce add in the fact that many counties, including my own (Monroe County), plan to rely on EMS to help vaccinate! The irony! As you see appropriate, please continue to drive this issue our numbers keep going up, our acuity is going up, and our people are getting sick. Our EMS system is already on the edge and I only hope we can hold on longer ---- EMS Colleagues- Within the last 24 hours, despite national recommendations and previous planning considerations, NYS is NO LONGER including EMS personnel in Tier 1a (First Distribution) availability for COVID Vaccine. Given our unique work environment operating in a confined space for extended periods of time with COVID patients there are no words for my frustration. Specifically Following the vaccination of 'high risk' health care workers, the priority will shift to all long-term and congregate care residents and staff, EMS and other health care workers. How EMS is not considered High Risk is beyond my comprehension. https://www.governor.ny.gov//governor-cuomo-updates-new-yo Please consider communicating to elected officials, specifically the Governor and Commissioner of Health, as well as any other elected leaders at the state, County, and even federal level, that EMS must be included alongside our high-risk hospital care providers else our already crumbling system will collapse. Attached is a toolkit from the AAA and a draft letter that you may edit and communicate to elected officials. Email, call, smoke signal. Get the word out. The need is now, this is moving fast, do not wait. Vaccine will be here in days and I fear we may have already missed the window to advocate for our own lives. Communicate to elected media, leverage media connections and use social media. The need is now although I fear too late as this rug was pulled out from under us. Jeremy T. Cushman, MD, MS, EMT-P, FACEP, FAEMS Associate Professor and Chief, Division of Prehospital Medicine Departments of Emergency Medicine and Public Health Sciences, University of Rochester Monroe County, City of Rochester, and Monroe-Livingston Regional EMS Medical Director Link to draft letter: https://www.dropbox.com//Draft%20Model%20Letter%20EMS%20Va Link to AAA Vaccine Prioritization Tool Kit: https://www.dropbox.com//AAA%20Vaccine%20Prioritization%20
