The anterior cruciate ligament (ACL) is an extremely important ligament in terms of overall knee integrity and stability. Specifically, the ACL ...connects the femur (thigh bone) to the tibia (shin bone) and is responsible for preventing anterior translation of the tibia or posterior translation of the femur. In this post, two videos from @michaelreiman demonstrating the lever sign for a suspected ACL tear are shown. The first video shows a negative test (no ACL tear) and the second shows a positive test. Labral tears involve an injury of the fibrocartilaginous rim that covers the periphery of the shoulder joint. These injuries can result in shoul...der instability, which describes a condition of increased joint mobility that affects the passive joint restraints. Labral tears can be broadly classified as traumatic or atraumatic. Traumatic cases result from an injury and are usually present on only one shoulder (unilateral) and demonstrate increased mobility in only one direction (unidirectional). Atraumatic cases, on the other hand, often present without an injury and demonstrate increased mobility on both shoulders and in multiple directions (multi-directional instability). Both forms of instability can cause pain and severely impair function, but can also respond to a conservative care program focused on reducing pain, avoiding stress to the capsule, labrum and/or shoulder ligaments and improving neuromuscular control. If you suffer from instability of the shoulder, make sure to give conservative care, specifically, neuromuscular control and muscle strength & endurance exercises a try before rushing into surgical reconstruction. Shown here are several exercises designed to improve neuromuscular control/stability of the shoulder complex, which can be beneficial when dealing with shoulder instability. Burkhead WZ Jr, et al. & Back pain following bouts of prolonged sitting is a common complaint. While we recognize that pain is complex and influenced by numerou...s variables, there are a few simple strategies that can be utilized to lessen back pain associated with sitting. While the best antidote to this situation is to spend less time sitting, this is not realistic for many people who must sit to do their work. So, besides trying to take more frequent breaks to get up and move around, try these simple strategies next time your back is acting up. 1Lumbar Extension: While in a seated position, the low back (lumbar spine) is held in flexion. So, stretching into the opposite direction (extension), can be relieving for many people. This can be done on the ground as shown here or in standing, if you can’t get on the floor. Extend your back 5 times and hold for a couple of seconds each time. 2Lumbar Rotation: Besides extension, rotation is often very effective for reducing low back tension and pain. Twist a few times each direction as shown here and hold for a couple of seconds. 3Kneeling Hip Flexor/Quad Stretch: Stretching muscles that connect the spine to the hip region also tend to work well. Additionally, the hip is held in flexion during sitting, so moving it in the other direction is likely to help. As the video eludes to, squeeze your abs and glutes to increase the intensity of the stretch. Shoot for 2-3 reps and hold for 15-30 seconds. 4Hamstring Stretch with Sciatic Mobilization: Again, stretching hip muscles that strongly influence the lumbar spine can be helpful. Try the standard hamstring stretch and add a sciatic nerve mobilization for a bit of extra help. The sciatic nerve originates from the low back, so mobilizing it gently can work to aid in keeping things loose and pain-free. Hold the stretch position and move your ankle as shown 10-15 times to mobilize the nerve and its branches.
