New York Therapy & Wellness 03.01.2021

In a year that threw all of us upside down, we really just want to say thank you to our amazing team. When the pandemic hit, NYTW therapists geared up and aided all that needed our help. We helped father's walk down the aisle of their daughters wedding, we assisted an 85 year old man be able to walk back up the stairs of his house so he can go to sleep with his wife again at night. They needed our help when help was difficult to get. To all NYTW clinicians and others around... the world, we thank you from the bottom of our hearts. Aside from our rehab team, our administrative staff upended their normal lives to make sure they can be there for all our patients. They worked day and night, making sure that whoever needed the help will get it. There were countless times this year where we weren't sure how we will make it through. We were clueless about the future. But the effort from entire NYTW family not only got us through but made us stronger. We ended our year helping more people than we ever did across more cities than ever before. We have assembled the best rehab team around and we're looking forward to what we've got planned for the new year. Wishing everyone a healthy, happy and successful new year. - Dan and Margo

Decrease your arthritic hand pain . Patients with arthritis may develop joint deformities, preventing them from successful using their hands to grip and grasp commons household objects. . Less range of motion is required to grip utensils with larger diameter handles, which in turn may reduce challenges for patients with limited ROM of the hand. ... . . Total Knee Arthroplasty . If you’ve tried everything else in the treatment of osteoarthritis [physical therapy, cortisone injections, PRP, gel injections] then you may be on the road to a knee replacement. .... The prospect of getting a knee replacement may seem scary , but if you take the right steps and find the right rehab specialist you’ll often feel even better than before! . Prior to a knee replacement your goal should be to improve your quadriceps strength and the ROM around the tissue around the knee - both of these will be reduced after surgery . . Immediately post-op you’ll usually receive at home PT services once’s discharged from the hospital. The focus here in these early stages is to decrease your pain and improve the motion of the knee ASAP. . Somewhere between 2 - 4 weeks your surgeon will advise you to go to outpatient care. Again, here our focus will be on increasing your mobility while now starting to transition to building strength and working on your performance of daily activities. . Working on strength is a long term commitment and will continue for the remainder of your rehab which will usually last til the end of your 3rd month or slightly beyond. Once discharged you should continue at a minimum a few strengthening & ROM exercises that your PT gave you for home exercise. . Occupational therapists scope of practice is centered around occupations. Occupations are the things you want to, and need to do every single day. . In your OT eval, your therapist will asses your engagement in the 8 areas of occupations. ADLs (activities of daily living), IADLS (instrumental activities of daily living), work, play, sleep, leisure, and social participation. . In geriatric home care, your OT will primarily focus on ADLs- dressing, bathing, toileting, hygi...ene, and eating. These are activities every person needs to do everyday, and many older adults lack independence or feel unsafe performing these activities. . Difficulty in performing ADLs is associated with diminished quality of life, poor self-efficacy, and high health care costs. When your rehabilitation treatment is focused on the activity itself that you want to perform better in, you will have a greater success in gaining your independence! . . . . . See more

. Patella Tendinopathy Rehab . Tendinopathy or what many people think of as tendinitis is a tricky injury to handle. Current research often links the cause of Tendinopathy with repetitive energy storage & release by the tendon or chronic overload. .... Tendinopathy will typically be treated by modifying the load going through the tendon in question. Modalities and manual interventions can be effective shorter term for pain modulation, but graded loading is where the bread and butter of this rehab lies. . As a clinician we need to make sure we are appropriately loading the tendon enough that we are strengthening the healthy tendon tissue that surrounds the disorganized collagen of the injured portion of the tendon. This needs to occur at the right dosage - too much load and we can send a tendon further down the road to dysrepair or degeneration. Proper patient education about what could be a lengthy rehab and about the acceptance of some pain when loading is critical in successful rehab of a tendon. . Remember with Tendinopathy we are more concerned with bolstering up all the tissue surrounding an area of dysrepair. We aim to treat the donut, not the hole which alludes to treating healthy areas of tendon. . Determining if tendon load is appropriate in rehab can be done with the 24 hour load response test. Essentially here a patient is performing a movement or action that is associated with tendon pain each day at the same time. Barring an increase in pain during our test we know the tendon is tolerating our doses load correctly. . Not only can falling be fatal in the elderly population, but even in cases where a fall is not fatal it can often led to a major surgery or a significant apprehension of movement can develop. .... PTs/OTs are not only trained to work falls prevention, but we can work with our elderly population to rebuild their confidence, strength, and balance after a fall.
