Traveler Form To help stop the spread of COVID-19, Los Angeles is requiring all travelers entering the city from another state or country to fill out this online form, acknowledging that they have read and understood California’s travel advisory. Travelers over the age of 16 must submit this form online prior to or upon arrival at Los Angeles International Airport, Van Nuys Airport, or Union Station. Failure to submit the form is punishable by a fine of up to $500. The State of California has issued the following Travel Advisory: The incidence of COVID-19 is increasing in many states and countries. Persons arriving in California from other states or Californians returning from other states or countries could increase the risk of COVID-19 spread. In addition, travel itself can be a risk for exposure to COVID-19, particularly travel through shared conveyance such as air, bus or rail travel. Travel Advisory for Non-Essential Travel [1] 1. Persons arriving in California from other states or countries, including returning California residents, should practice self-quarantine for 14 days after arrival . These persons should limit their interactions to their immediate household. This recommendation does not apply to individuals who cross state or country borders for essential travel. [2] [1] Non-essential travel includes travel that is considered tourism or recreational in nature. [2] Essential travel includes: work and study, critical infrastructure support, economic services and supply chains, health, immediate medical care, and safety and security.

New mandatory COVID-19 testing requirements for passengers traveling to the USA. Dear Valued clients, On January 12, 2021, CDC issued an Order requiring all air passengers arriving to the US from a foreign country to get tested no more than 3 days before their flight departs and to provide proof of the negative result or documentation of having recovered from COVID-19 to the airline before boarding the flight. This Order will go into effect on January 26, 2021. ... The U.S. government will require all passengers, including U.S. citizens, two years of age and older traveling to the United States from any international location to test negative for COVID-19.
