In this patient there is a significant left sided (the image on the right is a mirror image) disc herniation. Represented by the red arrow compression the left exiting S1 nerve which goes to the foot. This is a difficult case. There is also a rudimentary intervertebral disc at S1-2, which is a normal variant that is represented by the yellow arrow.

Post-graduate studies are a critical part of making sure your doctor is up to date and has a great network of other providers. This is a patient we had 2 weeks ago... 51 year old male with L5-S1 annular tear and disc degeneration. The annular tear is acute can the major cause of his pain. Incidentally we see Bone Marrow Hyperplasia throughout the spine. It is critical that it is differentiated FROM metastases in the spine cancer. What doctor they should be seeing is CRITICAL - DIAGNOSIS FIRST...they are stable and doing well...if this was something MORE OMINOUS they would have been send to the oncologist and PCP IMMEDIATELY.

This is a recent patient with a LARGE L5-S1central disc herniation. The red arrow points to it. The blue arrows show how the posterior longitudinal ligament is being stretched, normally it lays against the bone. That ligament is INNERVATED as is the DISC. That means that BOTH generate pain.

Here is a picture of a patient a few days ago with significant nerve compression at L3-4 and L4-5. I also show a NORMAL slice so you can see how much room the patient SHOULD HAVE for the nerves to come out. This patient is responding VERY WELL to treatment, it is the abnormal loading of the spine due to an underlying biomechanical problem that is deforming the disc. DIAGNOSIS and ASSESSMENT FIRST.
